Medical Clearances

All student-athletes can now access the medical clearance forms in Teamworks:
  • Log in at Teamworks.
  • Your username is your Oxy email address.
  • Your password is your Oxy ID#. (Returning student-athletes may have a different password if you changed it since your first log in.)
  • Returning Student-Athletes: if you don't remember your password you can reset it by entering your Oxy email address, click next, then click "Forgot Password."

There are a total of nine forms (all part of the Medical Clearance Forms Packet) to be completed and electronically signed:
  • Authorization for Disclosure of Protected Health Information
  • Concussion Responsibility and Baseline Testing
  • Consent to Treat
  • Health History
  • Insurance Information
  • NCAA Drug Testing Program and Medical Exceptions Procedures
  • Oxy Athletics Physical Exam
  • Release of Liability and Assumption of Risk
  • Sickle-Cell Trait Confirmation
Important Notes:
  • Medical clearance forms for all Oxy student-athletes are due by August 1, 2025.
  • If you are on more than one team, you have received duplicate copies of the forms. This is because Teamworks recognized you as two separate athletes on different teams. You only need to complete one of the forms. Do not complete duplicate forms. Pick one and fill that one out. For example, if you received two copies of the Health History Form, you only need to complete one of them.
  • Physical exams must be documented on our Oxy Athletics Physical Exam Form. Keep in mind these requirements when getting your physical exam:
    • Your physical exam form must be signed by a physician (MD or DO). Ideally, this physician should be your primary care physician. We cannot accept forms signed by a nurse, physician’s assistant (PA-C), or chiropractor. It is your responsibility to make sure that the form is signed by a physician before you leave the doctor’s office. If it is not signed by a MD or DO, you need to go back into the office and ask for the MD or DO to sign off on the form. We recommend that you confirm with the office at the time of making your appointment that a MD or DO will in fact be conducting the exam.
    • All sections of the physical exam form must be completed. This includes your height, weight, blood pressure, pulse, temperature, vision exam, and pupil assessment. If any of these measurements are missing, your physical exam form will be returned to you for correction.
    • One of the clearance status boxes must be checked by your physician.
    • The physician must provide their stamp on the form. If your physician doesn’t have a stamp, they can staple a business card in the box. 
  • All incoming (new and transfer) student-athletes will need to provide acceptable documentation of their sickle-cell trait status. Please refer to our Sickle-Cell Trait page for additional information on the types of documentation you will need to submit. Information on how and where to submit your documentation will be coming later in May.
